1. Complete the Lost or Stolen Key Report when an employee discovers and reports that he/she has lost or suspects that a key has been stolen at your site. Both the employee reporting the loss and his/her Principal or Supervisor must sign and date the form.
2. Use this form to indicate where re-keying is required (answer “yes” or “no”) and the reason.
3. Send the original Lost or Stolen Key Report to the Facilities, Maintenance, & Operations Department and keep a copy for the Department/School Key Log Book.
