1. Form LM-OP-40 includes the AGHS/NHS Off Campus Permit and Teen Driver Safety Act of 1997 information and is used at the secondary school level when a student is asked to run errands during the school day and uses his/her private vehicle.
2. The school site is responsible for ensuring that both sides of the form have been completed and that all required signatures have been obtained before permitting students to use her/her private vehicle for school related errands or activities.
3. Once complete, keep the original form at the school site and send a copy to the Business Office. |
