Emergency Cards:
Emergency Cards enable teachers and office staff to contact the appropriate person if a child becomes ill or has an accident. Please keep your records up to date with the correct names, addresses, and telephone numbers. This is vital to the safety and welfare of your child. Inform the school office and the school nurse of any changes of this information.

School Nurse:
Our school nurse is Sharleen Brooks. You may email her ( sbrooks@lmusd.org ) or leave a message on 805-474-3200, extension 2193. Office hour are from 8:00 to 3:30 Monday through Friday.
The nurse provides emergency care for injuries or illness that occur on campus. She will contact you, if necessary, using the phone numbers listed on your emergency cards in the office. The school nurse works closely with staff on students identified health concerns and provides a supportive school care plan to all teachers.  Vision and hearing screening are done annually for all special education and sophmore students. You may contact her if you have any concerns with you child’s vision or hearing.

If your child becomes sick or injured at school:
He or she cannot be released without the consent of a parent, guardian or other person listed on the emergency card.  Arroyo Grande high has a school based clinic run by the Community Health Center and is on campus Tuesday and Thursday from 8:00 to 4:00.
If, following an injury your child has a cast or brace and needs help to and from class, please notify the school nurse for accomadations. If your child is absent from school for more than three days, a note from the doctor is required. Please provide a doctors note if your child or absent for more than three days or if you have received a notice from the school about excessive absences from the school,  a note from your physcian or school nurse is required.

If your child needs medication at school:
Parents must bring the medication in its original container to school and complete a medication consent form for the school nurse. Emergency medications such as asthma inhaler, Epi-Pen and insulin pin and pumps are to be carried with students at all times while on campus.
